your corrective actions are creating tomorrow's problems
The standard response to a problem in quality is to add something. A new inspection step. An extra check. Another sign-off, another form, another control in the plan. Each addition feels like diligence, like the responsible answer to a defect that got through. And each one is a little heavier than the last. Stack up enough corrective actions over enough years and you end up with a process so encrusted with controls that the complexity itself becomes a source of new problems. The cure quietly turns into the disease.
the reflex to add
When something goes wrong, adding a control is the easy and defensible move. It shows you did something. It creates a record that a corrective action was taken. Nobody ever got criticized in the moment for adding an inspection. So the additions accumulate, one defect at a time, each justified on its own. What almost never happens is the opposite move, removing a control, questioning whether a check is still earning its place, simplifying a process that has grown top-heavy. Addition is rewarded and subtraction is risky, so processes only ever get more complex, never less.
complexity is where defects hide
Here is the problem with all that added control. Complexity is not free, and it is not neutral. Every extra step is another place for something to go wrong, another handoff, another judgment call, another opportunity to miss or mis-record. A process with fifty steps has more ways to fail than a process with twenty, no matter how well-intentioned each of the fifty is. The checks meant to catch defects become, in aggregate, a system so cumbersome that people start finding ways around it, or performing it mechanically without attention, or making errors in the very controls that were supposed to prevent errors. You solved one failure mode and manufactured three new ones.
the check nobody does anymore
There is a specific failure that complexity breeds. When a process has too many checks, people stop treating any of them as important. A single meaningful inspection commands attention. The tenth redundant check on the same routing gets a signature without a look, because everyone knows it is theater. So the pile of controls does not just add complexity, it dilutes the seriousness of the controls that actually matter. The critical check drowns in a sea of low-value ones, and the whole system trains people to sign without seeing, which is worse than not having the check at all, because now you have false confidence on top of the original risk.
corrective action should include subtraction
A healthier discipline treats every corrective action as a change to the whole process, not just an addition to it. When you add a control, ask what you can remove, whether this defect could be prevented by simplification rather than more inspection, whether a mistake-proof that makes the error impossible would let you delete three checks that were trying to catch it. The best corrective action often makes the process simpler, not more complex, because it eliminates the possibility of the defect rather than adding a net to catch it. That is harder and less obvious than bolting on another check, which is exactly why it rarely happens.
audit your process for scar tissue
Every mature process carries scar tissue, controls added in response to problems that may not even exist anymore, checks that made sense for a failure mode long since designed out, steps nobody remembers the reason for. It is worth periodically going through a process and asking of each control, what is this actually preventing, and is that risk still real. A surprising number will turn out to be defending against ghosts, adding cost and complexity and diluting attention while preventing nothing. Removing them is real improvement, even though it photographs as doing less.
Look at your most controlled process. How many of those checks are genuinely catching something, and how many are scar tissue that is quietly making the whole thing more fragile? The answer is usually uncomfortable, and usually worth acting on.
