Open enough corrective action reports and a pattern jumps out. The root cause is some version of operator error, and the corrective action is some version of the operator was retrained. It is probably the single most common corrective action in manufacturing, written thousands of times a day across the industry. It is also one of the least effective things you can do, because in the great majority of cases it treats a system problem as a person problem, changes nothing about the conditions that produced the error, and quietly guarantees the same defect will happen again.
retraining assumes the problem was knowledge
The logic behind retraining is that the person did not know how to do the job correctly, so teaching them again will fix it. Sometimes that is genuinely true, and retraining is the right answer. But most errors do not come from a lack of knowledge. They come from a process that made the error easy to make, a step that is easy to do out of order, a part that fits two ways, a condition where a momentary lapse produces a defect. In those cases the operator knew perfectly well how to do the job and made a normal human slip that the process allowed. Retraining a person who already knew how to do the task does nothing, because the problem was never that they did not know.
why it is so popular anyway
Retraining is the path of least resistance for a corrective action. It is fast, it is cheap, it closes the finding, and it locates the problem safely in a person rather than in the process or the design, which means it does not implicate any of the harder-to-change parts of the operation. It produces a signature on a training record that satisfies the audit and lets everyone move on. Every incentive around closing a corrective action quickly points toward retraining, and almost none point toward the slower, harder work of actually changing the process so the error cannot happen. So retraining gets written, the finding closes, and the underlying vulnerability sits untouched.
the tell that it did not work
There is a clear signal that retraining was the wrong corrective action, and it is recurrence. When the same defect comes back, often with a different operator, the retraining has told you plainly that the problem was never about that person's knowledge. A defect that recurs across different people is almost by definition a system problem, because the one thing those incidents have in common is not the individual, it is the process they were all working in. Yet the common response to a recurrence is to retrain again, or to retrain more people, which is doing the same ineffective thing harder and expecting a different result. The recurrence was the process telling you retraining does not address the cause, and retraining again ignores the message.
what a real corrective action looks like
A genuine corrective action changes the conditions that produced the error, not the person who was standing there when it happened. It asks why the error was possible at all and removes that possibility. It mistake-proofs the step so the part cannot go in the wrong way. It changes the sequence so the easy-to-miss step cannot be skipped. It fixes the design so the two similar components cannot be confused. These are harder, slower, and more expensive than a signature on a training record, and they are the only things that actually prevent recurrence, because they change the system rather than exhorting the person to be more careful in a system that makes carefulness fail.
use retraining honestly
None of this means retraining is never appropriate. When someone genuinely was not trained, or a procedure genuinely changed, retraining is exactly right. The discipline is to be honest about whether the error came from a real gap in knowledge or from a process that let a knowledgeable person slip, and to reserve retraining for the first while doing the harder work for the second. An operation whose corrective actions are mostly retraining is an operation that has mostly been treating system problems as people problems, which is why its problems keep coming back no matter how many people it retrains.
Look at your last several corrective actions. How many closed with retraining, and of those, how many addressed a real knowledge gap versus quietly blaming a person for a process that set them up to fail?
