you don't have a quality problem, you have a design problem
When a defect shows up in production, everyone looks at the floor. The process, the operator, the setup, the inspection. That is where the defect appeared, so that is where the investigation goes. But a large share of what gets labeled a manufacturing quality problem was not created in manufacturing at all. It was designed in, months earlier, by decisions made before anyone touched a machine. And you cannot manufacture your way out of a design that was never producible in the first place.
where the defect was really born
A part that is difficult to make consistently is usually difficult by design. A tolerance was specified tighter than the process can hold. Two features were placed in a relationship that makes them nearly impossible to achieve at once. A material was chosen that behaves badly in the process it has to go through. An assembly was designed in a way that lets it go together wrong. None of these are manufacturing failures. They are design decisions that handed the floor a problem it cannot fully solve, and then the floor gets blamed when the predictable defects appear.
the floor inherits what design decides
This is the uncomfortable part for anyone who has run production. Manufacturing lives downstream of design, inheriting whatever producibility the design left behind. A well-designed part is one a capable process can make reliably without heroics. A poorly designed one demands constant compensation, tight control, and a level of care no process can sustain forever. When the design is the problem, all the process controls and inspection in the world only manage the symptom. The floor can hold it together with effort and skill, but it cannot make a fundamentally hard-to-make part easy, because the difficulty was built in before it arrived.
why we blame the floor anyway
The defect appears in production, so production owns it, and the corrective action gets written against the process. This is convenient for everyone upstream. Design has moved on to the next program and is not in the room when the defect surfaces months later. The organizational distance between the design decision and the production defect means the two are rarely connected, and the people who could actually fix the root cause, the designers, never hear that their choice is generating scrap on the floor every single day. So the floor absorbs a problem it did not create and cannot fully solve, and the real cause is never touched.
the fix has to move upstream
The durable answer is to catch producibility problems while they are still cheap to change, which means in design. This is the entire point of doing design reviews with manufacturing in the room, of tools that force the team to ask how a part will actually be made and whether the process can hold what the print demands. A tolerance questioned in design is a keystroke. The same tolerance questioned after launch is a change order, a requalification, and months of scrap in between. The organizations that make good parts consistently are usually the ones where manufacturing has a real voice in design, early, before the decisions harden into a released print that the floor then has to live with for years.
a different question when defects appear
None of this means every defect is a design problem. Plenty are genuinely made on the floor and belong there. But when a defect is chronic, when the process fights the same issue no matter what controls you add, when good parts require your best people and constant vigilance, that is the signature of a problem that was designed in, not manufactured in. The useful question is not just how do we control this on the floor. It is whether this should have been this hard to make at all, and whether the answer lives in a design decision nobody is revisiting.
